Role Overview:

  • Clinical Care Coordinator role situated at Diamond Creek Care Community
  • Permanent Full-time position

Primary Duties:

  • Effective leadership and mentorship of care teams within the facility under the guidance of the Care Manager
  • Oversight of education and training for both new and existing team members
  • Assessment of quality care performance metrics, clinical records, work protocols and resources to ensure or surpass compliance standards
  • Guaranteeing personalized care for residents, including the development, execution and review of care plans on a regular basis
  • Enhancing clinical practice proficiency in areas such as medication management, wound care, continence management, palliative care and dementia care
  • Supervision of the ACFI process

Qualifications:

  • AHPRA Nursing Registration and experience in Aged Care
  • Alignment with Opal’s Values: Compassion, Accountability, Respect and Excellence
  • Preferably a background as an RN Educator
  • Thorough understanding of Aged Care standards and their practical application
  • A drive to enhance clinical care through coaching and team advancement
