Role Overview:
- Clinical Care Coordinator role situated at Diamond Creek Care Community
- Permanent Full-time position
Primary Duties:
- Effective leadership and mentorship of care teams within the facility under the guidance of the Care Manager
- Oversight of education and training for both new and existing team members
- Assessment of quality care performance metrics, clinical records, work protocols and resources to ensure or surpass compliance standards
- Guaranteeing personalized care for residents, including the development, execution and review of care plans on a regular basis
- Enhancing clinical practice proficiency in areas such as medication management, wound care, continence management, palliative care and dementia care
- Supervision of the ACFI process
Qualifications:
- AHPRA Nursing Registration and experience in Aged Care
- Alignment with Opal’s Values: Compassion, Accountability, Respect and Excellence
- Preferably a background as an RN Educator
- Thorough understanding of Aged Care standards and their practical application
- A drive to enhance clinical care through coaching and team advancement
